Although we are contracted with the following insurance companies, some HMO plans exist under these plans that we do not participate with. Please contact your insurance company to verify your eligibility for services with our physicians prior to your appointment.

Fees and Payments
We request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. If payment is not received, we reserve the right to refuse future appointments on delinquent accounts. If your account becomes past due, we will take necessary steps to collect this debt. All unpaid accounts for which payment arrangements have not been made are subject to collection procedures and additional collection costs.
Co-Payment/Co-Insurance and Filing Claims
Our office will file all insurance claims with all insurance companies. Once we receive payment or non-payment notice from your insurance carrier, any balance remaining will be billed directly to you. We require co-payment or coinsurance payments at the time of service. We accept MasterCard, VISA, Discover, cash, and checks.
Self-Pay, No Insurance?
For patients who have no insurance, payment in full will be required at the time of service.
